The Homemaker is expected to carry out routine assignments independently within the Agency’s program and procedures and within his/her scope of responsibility. As a Homemaker for our agency, you will provide essential services to elderly clients or individuals with disabilities, a chronic illness, or other health issues. Our ideal candidate is patient, independent, and self-sufficient, who enjoys taking care of others.
Full-Time Registered Nurse
A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Clinical Services.
Licensed Practical Nurse (LPN)
As a Licensed Practical Nurse with our agency, you will deliver compassionate, skilled nursing care to clients in the comfort of their own homes. The ideal candidate is experienced in home care, knowledgeable in using WellSky, and capable of working both independently and collaboratively. Bilingual applicants are highly preferred. HHA – Direct Care Aide
In Rivers of Hope, consisted of a multidisciplinary team the director, registered HHA, care manager and administrative manager provides the highest quality of services and advocacy for children and adults with intellectual, developmental, and physical disabilities. We offer monthly or frequently home visits. We also provide 24-hour coverage for emergencies.

Job Description & Duties
- Provide personal care and assistance to clients in their homes
- Assist with activities of daily living, such as bathing, dressing, and grooming
- Administer medication as directed by healthcare professionals
- Monitor and record vital signs, such as blood pressure and temperature
- Assist with medical documentation and maintaining accurate records
- Help clients with special needs or conditions, such as mobility issues or cognitive impairments
- Prepare meals according to dietary restrictions and assist with feeding if necessary
- Perform light housekeeping duties, including cleaning and organizing client’s living space
- Provide companionship and emotional support to clients
Qualifications
- High school diploma or equivalent
- Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) preferred
- Experience working in a home health or similar setting is a plus
- Knowledge of medical terminology and basic medical procedures
- Ability to follow instructions and work independently with minimal supervision
- Excellent communication skills and the ability to interact with clients and their families in a compassionate and professional manner
- Strong organizational skills and attention to detail
