Job Summary

A Case Manager at Rivers of Hope is a compassionate and organized professional who coordinates and oversees the delivery of high-quality home care services. The Case Manager works in partnership with clients, caregivers, interdisciplinary teams, and external providers to ensure person-centered care plans are developed, implemented, monitored, and adjusted to meet clients’ evolving needs. This position involves regular communication, timely documentation, and collaboration to ensure optimal outcomes for the individuals we serve.

Qualifications

  • Bachelor’s degree in social work, Nursing, Human Services, or related field (preferred).
  • Minimum of one (2-5) years of case management or care coordination experience, preferably in a home care or healthcare setting.
  • Must have a valid driver’s license and reliable transportation.
  • Must pass an OIG, and Criminal background check.
  • Strong organizational and interpersonal skills.
  • Proficient in using computers, EMR systems, and Microsoft Office tools.

Responsibilities and Essential Functions

  • Conducts comprehensive assessments of clients in their homes, including psychosocial, medical, environmental, and functional status.
  • Develops individualized service plans in collaboration with the client, caregiver, and interdisciplinary team.
  • Coordinates home care services including homemaker, personal care, and nursing as appropriate.
  • Ensures timely and accurate documentation of all client encounters, services, communications, and progress.
  • Monitors the delivery and effectiveness of services, adjusting care plans as needed.
  • Acts as a liaison between families, service providers, and community resources.
  • Communication changes in client condition or service are needed by the appropriate internal or external parties.
  • Provides support and education to clients and families regarding available resources and benefits.
  • Ensure compliance with agency policies and state/federal regulatory requirements.
  • Participates in interdisciplinary case reviews, team meetings, and quality improvement activities.
  • Assists with clients on boarding, intakes, reassessments, and discharges.
  • Maintains confidentiality of all client and agency-related information.
  • Responds to emergencies, urgent client concerns, or unexpected events promptly and appropriately.
  • Provides occasional supervision and training to administrative assistants or interns as assigned.
  • Other duties as assigned by the Director of Care Services.
