Rivers of Hope provides a range of in-home care services, including: 

  • Adult Foster Care (AFC): Support for individuals aged 16 and older with developmental, intellectual, physical, or mental disabilities, allowing them to live in a family-like setting with trained caregivers. r 
  • Group Adult Foster Care (GAFC): Personal care services within community or assisted living environments for elderly and/or disabled individuals.  
  • Home Health Care: Medical and non-medical assistance in individuals’ homes by skilled professionals, including medication management and daily activity support.  
  • Private Pay Services: Customized care plans for clients who prefer to pay out-of-pocket for services, offering flexibility and personalized attention.  
  • Medication Administration Program (MAP): A referral resource that connects individuals with training programs designed to equip staff with the knowledge and skills to safely administer medications in compliance with healthcare regulations. Note: This training is provided by external partners and not conducted in-house by Rivers of Hope. 
  • Veteran Care: Specialized services honoring veterans, ensuring they receive the support and recognition they deserve. 

2. Who is eligible for Adult Foster Care (AFC) services? 

Individuals aged 16 or older who have developmental, intellectual, physical, or mental disabilities and cannot safely live alone may be eligible for AFC services. Eligibility is determined through assessments and coordination with healthcare providers.  

3. How does the Group Adult Foster Care (GAFC) program differ from AFC? 

While both programs provide personal care services, GAFC is designed for individuals residing in group settings, such as assisted living facilities, offering support within a community environment. In contrast, AFC involves living with a caregiver in a family-like home setting.  

4. What is the Medication Administration Program (MAP)? 

The MAP is a training initiative ensuring that staff members are equipped to administer medications safely and effectively, adhering to healthcare regulations and standards. 

5. Do you offer services for veterans? 

Yes, Rivers of Hope provides specialized care for veterans, recognizing their unique needs and ensuring they receive the support and respect they deserve.

Rivers of Hope serves various counties, and communities. We serve the Plymouth, Bristol, Norfolk, Suffolk counties. And some of the communities including Brockton, Boston, Braintree, Avon, Randolph, Abington, Dorchester, Malden, and surrounding areas. For specific service areas, please contact us directly.

8. Is there paperwork involved in starting services? 

Yes, initiating services involves completing necessary documentation.

Our culturally competent team includes members who speak English, Haitian Creole, and Cape Verdean Creole, ensuring effective communication with diverse clients.

11. What are your office hours? 

Our standard office hours are Monday through Friday, 9:00 AM to 5:00 PM. We offer services outside of business hours, including weekends and holidays, based on client needs. Please contact us to discuss specific scheduling. 

12. Do you accept insurance or MassHealth? 

Yes, Rivers of Hope accepts MassHealth, the Medicaid program in Massachusetts, as well as other applicable insurance plans such as VA CCN, UHC, Tufts and others. For private pay clients, we also offer flexible payment options. If you’re unsure about coverage, our team can help verify eligibility and assist with enrollment. 

13. Do you provide 24-hour care or overnight support? 

Yes, we offer 24-hour care and overnight support based on client needs. These services are available through both private pay options and qualified programs. Please call us to discuss your situation and care requirements. 

14. Can I choose my caregiver or switch if it’s not a good fit? 

Absolutely. We match clients with caregivers based on needs, preferences, and personality. If for any reason you or your loved one are not comfortable, we will work with you to find a better match. 

15. What is your caregiver screening and training process? 

All caregivers undergo thorough background checks, CORI clearance, reference checks, and in-depth training. We also ensure ongoing education and supervision to maintain the highest standards of care. 

16. How quickly can services start? 

In many cases, services can begin within a few days, depending on the urgency, program eligibility, and completion of required assessments and paperwork. Call us to discuss your situation and timeline. 

17. Are services available to people living in their own home, or only with caregivers? 

We provide care in a variety of settings including the client’s home, a family member’s home, or with a live-in caregiver (for AFC clients). We tailor each plan to best meet the client’s living arrangement and support system. 

18. What’s the difference between private pay and insurance-funded services? 

Private pay services are paid out-of-pocket and offer more flexibility in service types, hours, and customization. Insurance or MassHealth-funded services follow specific eligibility criteria and care plans based on program guidelines. 

19. Do you offer respite care? 

Yes, we offer respite care for family caregivers who need temporary relief, and in are other programs as well. This can range from a few hours to overnight care. Contact us to learn more about availability and options. 

20. Do you offer companionship or homemaker-only services? 

Yes, Rivers of Hope offers non-medical support like companionship, light housekeeping, meal prep, medication reminders, errands, and transportation assistance under both private pay and qualified programs. 

21. I’m a provider or case manager. How do I refer a client? 

You can submit a referral directly through our website or by calling us at 508-857-0629. We collaborate with providers, discharge planners, and community partners to ensure a smooth onboarding process.
